IMT Application

1. Application Steps

You may, at your convenience, fill out the IMT Application and Forms electronically and E-Sign. In accordance with the Uniform Electronic Transactions Act (UETA) and the Electronic Signatures in Global and National Commerce Act, or E-Sign and other applicable local or state legislation regarding Electronic Signatures and Transactions, the Applicant and IMT agree to the use of electronic signatures as a legally binding method of signing this Agreement and Forms. The parties hereby agree that either party may electronically sign by utilizing a digital signature. If, however, you prefer to fill out the IMT forms manually, please print, sign and send your completed forms to IMT at our address provided below.

2. Physician’s Certificate

In order to participate on an IMT guided trek or climb, we require you to have a physician’s certificate and to submit that certificate within 60 days of your Trek/Climb.

3. Passport Information

Please provide IMT with a color copy of the first two pages of your Passport which shows your name, signature, date of birth, passport number and citizenship. Please insure that your Passport is valid for no less than 6 months after the trip.

4. Payment Information

IMT is pleased to offer our clients 2 quick and easy online payment options:
Electronic Funds transfer which allows you to pay directly from your bank with no additional fees;
or by Credit Card. Please note that A 3% surcharge will be applied to all credit card transactions. Debit cards are not accepted.

6. Notification

We will notify you by email when we have received your completed Application and that you have been accepted for participation in an IMT trek/climb. We will send you additional instructions and updates to your email address.

7. Insurance

All IMT participants must obtain travel insurance that includes medical evacuation and medical expenses for the duration of their IMT program. Any IMT participant going on a climb must also have an insurance plan that covers high-altitude mountaineering. Here is a link to an approved plan which we recommend:

Global Rescue provides coverage for medical evacuation, travel insurance for incurred medical expenses and trip cancellation. In order to participate on any IMT trek or climb, you must enroll in the membership plan and purchase travel Insurance within 20 days after your application and fee submission.
